This agreement gives Launch Hub Learning permission to include your child in its programs, activities, and events while outlining our commitment to maintaining a safe and enjoyable environment.

Consent for Participation and Medical Authorization

As the Parent/Guardian, you give your consent for your child to participate in all Launch Hub Learning classes, activities, and events. You understand that these activities may include physical activities. Launch Hub Learning is committed to taking all reasonable measures to ensure the safety and well-being of your child. In the unlikely event of an emergency, you authorize Launch Hub Learning to seek medical care for your child if necessary and agree to be responsible for any medical expenses incurred.

Release of Liability

By agreeing to this waiver, you acknowledge that Launch Hub Learning, its employees, and representatives will take every precaution to provide a safe environment. You release Launch Hub Learning from any claims related to your child’s participation in activities, with the understanding that all reasonable care and supervision will be provided.

Code of Conduct

You agree that your child will follow the behavior guidelines established by Launch Hub Learning to ensure a positive and respectful experience for all participants. Inappropriate behavior may lead to disciplinary action, which could include removal from activities if necessary.

Governing Law and Entire Agreement

This Agreement shall be governed by the laws of the State of Virginia. This Agreement constitutes the entire understanding between you and Launch Hub Learning and supersedes all prior agreements regarding the subject matter contained herein.
